Did you lose your Qualification, Statement of Attainment, or Card?

If you have lost your Qualification, Statement of Attainment, or Wallet Card, you may request a replacement by submitting the Award Replacement Form (please click on the “How do I get the replacement?” tab).

A replacement fee applies for each Qualification, Statement of Attainment, or Wallet Card:
- $30.00 AUD (including GST) – PDF version + printed copy
- Free – Just the PDF version
