Award Replacement Request

A replacement fee of $30.00 (including GST) applies for each Qualification, Statement of Attainment, or Wallet Card.

You are required to provide two forms of identification (preferably photo ID), such as a Driver’s Licence, Passport, or Proof of Age card when you fill out the form and picking up the award.

Once payment and all required documentation have been received, replacement awards will be processed and issued within 10 business days.

If the award is to be collected by a third party, written authorisation from the student is required. This can be provided via a direct email or a signed letter from the student, which will be verified prior to release.
